CHHA RN Manhattan
HOURS: M-F 9-5, one Saturday a month
SUMMARY: providing CHHA care to patients in a specific area must complete 30-35 points per week
DUTIES:
Administer nursing care to recovering patients
Diagnose and establish patient treatment plans
Monitor and report changes in patient symptoms or behavior
Communicate with collaborating physicians or specialists regarding patient care
Educate patients about health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and medical facilities
Maintain accurate patient medical records
Provide advice and emotional support to patients and their family members
Regularly evaluates patient nursing needs
Initiates the plan of care and necessary revisions as patient status needs
Provides health care instructions to the patient as appropriate per assessment and plan of care
Identifies discharge planning needs as part of the care plan development and implements prior discharge of the patient
COVID: Required, medical exemption accepted
QUALIFICATION: RN, 6+ months CHHA and Oasis and wound care, car and DL experience required
